This series consists of materials on the support services offered by GCHP. These include blank copies of the multitudinous client forms used over the years; correspondence, staff and volunteer information, photographs and negatives, and information on the Worried Well Program, as well as the Living Well project, which latter replaced GCHP as the group's name.
